Returning after multiple injuries

All,
I'm a 54 year old male, who three years ago had reached a fairly high standard of fitness. I was running 2 or 3 marathons a year (sub 4 hrs), numerous sportives and multi day cycling events and a handful of triathlons.
Since then I've had multiple injuries which have stopped me training at the level I used to. Initially I had an operation to resolve ab fib, which was successful but I still get the occasional episode where my heart rate goes very irregular. Post op I started running again and probably due to 'too much, too soon' I wrecked my left Achilles which took about 9 months to get fixed and so it was last Christmas when I started again.
I was doing okay, taking it a bit easier and building up my running when I twisted my right knee (not running related) at the start of the summer this year and needed surgery to repair the cartilage this September. I'm just starting to run again.
During all of this, I've kept cycling, but mostly at a fairly easy pace and am now looking to start ramping up my training but am cautious about going too hard.
Does anybody know of any resources that I could refer to to try and get back to some decent level of fitness ? I'm thinking of either books or maybe a dedicated personal trainer who has some experience in this area.
I don't plan to get back to running marathons (I've been told not to after the heart issue) , but would like to get to running halves, and back into the sportives and maybe some trail running.

i've worked with quite a few athletes on paralympic squads etc, and also worked with a few with a variety of issues from cardiac to auto-immune conditions.
