I have had lateral ligament pain in the past and its enough to make you dread riding. It's a set up issue. I have put it down to saddle fore aft position and the relationship of cleat positioning and saddle height. When your on the mtb do you have your cleats set back towards the midfoot? I found that I'm more comfortable with the cleats set further back from the toes. My Mtb shoes allow this as the 2 bolt rail has enough adjustment, I use time attac pedals. For the road bike i use speedplay zeros with alloy rear extender plates on standard 3 bolt soles, having plenty of pedal float should also help.Caveat - I buy and ride cheap, however, I reserve the right to advise on expensive kit that I have never actually used and possibly never will0

I run flats on my mountain bike, I would kill myself if I ran SPD's the way I ride MTB. (bull in china shop)
Took the bike out again last night, along my usual route. Took my ISP spacers and allen key out just incase. This may sound like an exaggeration but I can promise you it is not... smashed my PB's up all the notable climbs and when I got home my legs felt golden. (maybe I was on a good day)
Nothing more to add other than the bike fit I had when I purchased my bike was rubbish.0 -
