How does foot measurement affect saddle height? For example, the same person with the same measurements but with a foot 42 and a 45 foot foot how much should he change his height in the second case?

It's a little trial and error as it also depends on flexibility and how you pedal.

He therefore assumes that he is the same person, so he has the same flexibility. Between a foot size 42 and a 45 of how much should the saddle be raised? Lemond formula 0.883 does not account for the length of the foot

The lemond formula is a starting point like most other ways of setting the height. For example heels on pedals and peddling backwards without rocking your hips. Then adjust up or down till it feels right.
