Pain underneath right ribcage

Hi,
I've been struggling with a stabbing pain directly under my right ribcage when I'm trying hard on the bike (e.g. 170+ BPM, making a sustained effort to breakaway in a race etc), for around 4 years. I raced for one year without problems, but the past 4 years have been ruined somewhat. Everytime it occurs, I'm forced to reduce effort until it subsides. The best comparison I can make is that it feels like stitch, only it isn't.
A few observations I've made -
1. I can ride on the turbo doing high intensity intervals (300+ W, 180BPM+ 4+ minutes) without any issues at all. Infact any turbo session with equivalent hard efforts to my road riding is absolutely fine. Same turbo bike position as my road bike.
2. On windy days, I seem to get the pain more frequently/easily.
3. I don't feel sick or feel ill.
Any observations or advice would be appreciated as it is a real nuisance.

I have had the same issue for the last 6 months. Will respond properly later but have to dash off to a related CT scan in 5 minutes!0

Obviously any chest pain is worthy of a visit to a professional, but it sounds a little like a muscle cramp in your obliques. Does it feel like its going in to spasm?
Some broom handle stretching and core work might be worth a try. side raises, side plank side crunches etc.0 -
Might be a chronically pulled quadratus lumborum muscle. I reinjure mine every now and again.
