In the marine world there is the Marine Accident Investigation Branch (MAIB) - it is their job to investigate accidents and near accidents in the marine world in a neutral manner. They are not there to issue blame, they are there to help everyone understand what went wrong, why it went wrong and how we can all learn from it.
The same is true of Aviation. Many of the reports are publicly available and make fascinating reading. The level of detail they go into is immense. They do have the advantage of significant data logging and procedures to fall back on. As far as I know reports on road accidents are not available publicly which makes analysing the accident statistics hard because we really don't know what happened and no one can learn from an incident. At the very least publish a "Top 10 ways people get killed / seriously injured on bikes" to give us some clues.
  • The problem is that the numbers are thankfully very low partically for London, which is 15 deaths a year on average.

So you get big % swings if the numbers are up or down, HGV are a constant factor though again the % caused by them swings up and down.

Couple in the huge rises in numbers of cyclist and types of cyclists as a group we are not uniform. Best guess terrorty though HGV plus bike is sound
