Hello,
I am looking for some help from the cycling community, I am co-ordinating a study that investigates the correlation between power output and cyclist demographics. I have put together a short survey, and I would be very grateful if you could take the time to fill it in – it shouldn’t take more than a couple of minutes!

Done.
But it's a slightly weird premise for a survey, and the demographics questions were quite limited, height, weight, ethnicity and a couple of questions about health and diet. Can I ask what particular questions/relationships you are interested in?
I also suspect that simply asking people for their FTP power figure is not going to produce very reliable data.. ;-)0 -

Another flaw, the options for time per week of exercise are too low, I mean, anyone who knows what their FTP is will generally be doing more than 6 hours a week. You'd be better off sliding the scale upwards, and removing the no exercise and 1-3 hour options,
Of the questions you've asked, training time will have the biggest impact on FTP, and you've not allowed a great enough range of answers to give any meaningful information.0
