Foot Pain whilst riding

Hi,
My mate has been experiencing pain in his feet. He reckons he can't feel his toes if he rides over an hour. I'm riding to France with him for the tour in July and don't want him bitching the whole way!
He's tried loosening the straps on his shoes but it doesn't help.
Any ideas what might be causing this?
He has proper road shoes with Look pedals/cleats.
Apologies if this has been discussed before, I tried to search for the topic but it came up with every post ever written since the dawn of time.

Positioning of the cleats? Are they on the ball of the foot?
Seat position...is he sat comfortable
Clothing "too" tight. Yes I realise lycra is tight but is he wearing the right fit.

Shoes too small? or as roscoe wrote! Most likely a shoe/cleat problem as its in both feet.0
