It doesn't look photoshopped to me. It looks like it's penetrated just lateral to the tibia. If you check your lower leg then you'll quickly realise that the big bony part is to the inside of your leg and the muscle/flabby part is towards the outside. There's a fair gap between your tibia (shin bone) and fibula (other side of your ankle) and that bit of wood is smack, bang in the middle. You wouldn't need to fracture anything to get that.
