Cable tension or dirty cables.
Adjust the cable tension either at the shifter end, or the mech end.
Clean the cables by shifting the mech into the smallest cog (without pedalling) and you should be able to pull the outer cable from the cable stops, enabling you to clean the inner cables.

Or the hanger is bent. stand behind and check the derailler arm is parallel to the cogs
