Rear wheel probems

right my shred runs v8 rotors front and back and when running they run superbly, despite some people calling them overkill but im a big chap and i do alot of freeride on it so like the hayes stopping power, however recently the rear disc has started rubbing on the frame making a horrid grinding noise, so i spaced out the axel with another washer giving the rotor clearance, then to find i needed to space out the caliper aswell, i did this to find the wheel wasn't running very freely - any suggestions on how to solve the total problem without having to down grade the rotor and mount'
and recently my rear axel has been slackening off causing a very unsafe[xx(] wheel - any suggestions (its a shimano deore rear hub)

ok the first problem was that you needed to reset the bearings in the hub NOT start spacing things out.
SORT out the hub and put stuff back to normal. read parktools if you know as much as it seems.

thanks, i tried this at the weekend, the bearings are fine the axel is still straight and sat evenly between both ends,i re-greased everything, i realise my make shift solution isnt ideal as it shifts the wheel towards the one side, but fail to see that when i set it up in the shop it ran fine up until this last week - im going to have to take the spacers out and possibly reclamp the frame stay again

ok, so i followed up what nick said, but it hasnt helped, the only way i can see round it is that i either clamp the stay and basically squeeze it slightly so it gives clearence or i space out the end bolt on the disc side to give clearance (but this would move the wheel out slightly which i know isnt ideal)
its probably something really basic but i just cant seem to understand how to sort it, ANY help would be greatly appreciated
