all the gears a shafted on the stab, and they keep jumping between gears, also the chain keeps jumping off, i *thought* that i may be running the chain wrong around the chain device, but sounds like my rear mech either needs adjusting or has AIDS

could be a worn chainring or a stretched chain if its jumping off at the chain device, If its jumping off on the cassete its probably worn or badly adjusted in that area.

I'm going to put on a new gear cable next week, and also a new chain, if that no works i'll get the lbs to check chainrings or cassette and see if either need replacing.......... worst case is new rear mech i think [V]
