The top cap external diameter inside the fork is around 18mm and the for internal around 22 so how does it reinforce the fork when tightening the top stem bolt?

The top steerer bolt is to pull everything together before tightening the stem bolts ,it's not supposed to reinforce anything .

It's only there after the stem is tightened to cover the hole

Thanks Trevor, it seems strange that the top stem bolt could crush the steerer as it’s not supported and the instructions above say the top cap provide critical support to the carbon steerer when tightening the stem bolts!

Get an old carbon steerer and try to crush it by tightening the stem bolts.

    Good luck.

Once you tighten the stem bolts that topcap will come off ,the wording suggests it compresses the steerer to it .That's not the case .

The Si system made it more complicated than it needed to be ,same as Cannondale cranks and the special tool needed to remove them .
