How does this even happen? How does he know where he is to spot the landing? I just don't understand how his head can understand where he is in the trick.

I suppose it's no different to the divers or ski aerial skiers who are putting in multiple twists and turns prior to hitting the water / snow perpendicular and in the right plane - it's all about knowing what forces are required to get the move going, committing to it mid flight rather than baling, and being confident that your final move will start to bring you out in the right orientation. It's gymnastics on a bike.
None of which understates how incredibly talented these guys are, and how hard they must practice to get this stuff nailed. You could see how "pumped" he was at the execution to know that he nailed that one in the comp as best as he thought he ever could.2015 Canyon Nerve AL 6.0 (son #1's)

Not sure about the broken bones, you can take massive crashes on dirt jumps without doing yourself any real damage. I've crashed on a huge double at my local (Windhill bike park) several times in a row trying to learn to suicide. As long as you know when it's gone wrong, you can bail and ditch the bike. On step ups it's even better because the landing isn't far below the highest pint of your jump, you can almost (but not quite!) step off the bike and on to the lipTransition Patrol - viewtopic.php?f=10017&t=130702350
