Bought a Sram Red crank off eBay, turns out it's standard size rather than compact which is what I'm used to but I'll give it a shot, I don't climb a lot and its for a spare bike...I think I have a medium cage RD...will this do or do I need a Large?

There is no such thing a large RD. There is short cage (designed for a 23,25,28t cassettes) and medium cage (designed for a 32t cassette). If your bike came with a 32t cassette then its probably a medium cage.

You will most likely need a longer chain to use the 39/53 chainset.

The medium cage will work fine with that chainset as long as the chain is long enough.

I use a 53/39 sram red chainset and 11-32 cassette with a medium cage rear mech , surprising how long the chain needs to be though I just fitted a new chain and only took about 2-3 links out of it as far as I remember
  • If you don't climb a lot just stick an 11-25 or 11-28 on there.
