    So it seems - but my other bike, also a PX with the exact same BB, has done well over 10,000 miles, quite a lot of them in the Scottish rain, and has no problems. I would just put it down as a YMMV thing if it wasn't for the complete lack of noise and resistance from the BB in question.

    This. Not tight enough. The pedal/crank interface isn't a great design. It should be like modern car wheel nuts really where the contact has a tapered conical face.

    OK - so it turns out there's a little shim sort of thing goes over the axle on the drive side... it was still sitting on the old cranks which fortunately I didn't throw out. Not sure why I couldn't find a new one on either the new BB or crankset... anyway, job done. And maybe Dennis was right...

    PS: any significance in the fact that the BB manual is first in SRAM's service page quick links?
