adjusting gears?

Changing inners and outers on my gears but got myself a little confused with the adjustments, anyone any points, this is what I understand the process is (after a lot of head scratching)
The derailleur is a shimano deore LX rear...
feed the inner through the shifter
Turn the cable adjustment screw on the shifter fully in then one turn out
Lock the nut at the derailleur with the chain at the smallest cog
Turn the derailleur adjustment screw in then out one turn
Check the high/low points
then check smooth changing, turn the adjustment screw to alter.
any help/pointers welcome.
I'm also not sure what adjustment screws to use i.e the one at the shifter end or the one at the derailleur?
