Are they all the same apart from length? Or does the one for internal wiring of the chainstay to the rear mech (suffix SD50-I) differ from the one for external wiring (suffix SD50)?

AFAIK, they are all the same. I've bought a few in my time.

The external stuff just has extra items to stick the cables to your frame.

That's not quite correct. Certainly for the original Di2, 7090 the internal and external wiring is different. The internal version has a junction which all the cables coming from other components plug into then it gets inserted up into the downtime through the bottom bracket. The plugs on this fit through a 4.5mm hole. You can't do this with the external one as you would have to pass the plugs for the derailleurs etc in from the bottom bracket and they're much to large for the holes in any frame I've seen - not to mention the battery connector!
