Brakes going off centre

I'm having a problem with my 2010 Force front brake caliper. I am using a 13 mm wrench to hold the centering bolt while I tighten up on the fixing bolt with an allen key. I am securing them tightly in my opinion but they seem to go off centre fairly easily once in use. Do I just need to tighten the fixing bolt more or is there anything else that could cause this ? My front wheel is perfectly true by the way.

    Does the nut acually tighten the brake or does it stuck on the end of the threads?
    In the last case you need a shorter nut (current is too long for the fork thickness).
    Happened to me once.

    That's something I have not considered. Will have to check.

    If that's not the problem , maybe a special washer with knurled surface between brake and fork is missing (don't know much about SRAM), Campanolo has such things and they work....
