I was sitting watching TV last night when I heard a loud escape of air. This is what I found when I checked the bike this morning. Does anybody have any idea what caused it? The inner tube is about 3 years old.

Looks like the rubber surround has failed and it's been pushed out by the air pressure, photo from the other side would be better, it maybe the rubber surround was damaged movement inside the tyre and it being cut by edges of the hole in the rim.

Just fit a new decent tube.
