Grainy headset.

My headset feels very grainy and not at all smooth when I turn the bars. I have tried regreasing, but it is still there - is it time for a headset replacement?

Depends. Can you feel it when riding and does it affect your ride?

It doesn't affect my ride at all.

So nothing to worry about then. You can't ruin anything else apart from the headset.

Can you take out the headset, clean and replace bearings? Probably cheaper than new headset
