Colnago CLX 3.0 headset creak and loose

Can anyone help with technical details on the Colnago CLX 3.0 headset. Is there anything unusual about it? I have a recurrent and loud creak from the headset that seems to work smoothly enough. But I am puzzled why it loosens on a regular basis. I slacken the stem bolts, and adjust the top bold gently to solve the problem, but the creak remains and the forks loosen again after a few hundred miles. Am I missing something?

    I cannot recall if the frame has integrated headset?
    It may be the frame or fork is not faced properly, and the paint in compressing.
