KEO carbon pedal came off spindle!

Hi there, I was riding today and right shoe pedal interface felt wobbly, I slowed down expecting to see the cleat had come loose from shoe and instead the cleat was solid and the pedal flew off the spindle!
I managed to push it back on and it came off twice more when working hard out the saddle, couldn't see if anything had broken but suspect something snapped inside since the axle comes with the pedal as standard.

I found one guy had same issue on another forum few years ago but no real answer...?

    Never seen a keo come apart like this before but I've seen a few similar ones. How has it come apart? Has it unscrewed from the crank arm end of the pedal body? Or are the threads (into the plastic body) damaged?

    Pedal coming off and you then worked hard out of the saddle?!! :shock:

    There's no damage to the thread and that bits secure, it seems below that where there no thread, and looks like just pushes on with a click but don't know if something snapped and there shouldn't even be a join there. When I said work hard out the saddle I could if don't push sideways or lean the bike

    I've had ths on a Keo2 but it was only the axle unscrewed from the pedal body? I simply screwed it back
