creaking and squeaking when pedalling

I've got pedals like these:

on my cube commuter. I'm noticing 2 noises while pedalling that I can't replicate off the bike. First is a crunchy grindy noise when I pedal, second is a squeak, again when pedalling.
Neither noise will be particularly noticeable when I start my (10 mile) commute but by the end it sounds like a load of mice doing metalwork.
I've really done maintenance on them so my questions are:
1. Is it likely I'll need to replace the bearings or just grease them? There isn't any noise at rest or any play?
2. The creaking seems like a worn cleat issue, is this likely to be on the right track. I've probably had these cleats for 4 or so years.
