bearings,behind star nut in fork steerer???

Basically, mate is servicing his headset. He has text me to say that there are loose bearings,within the steerer behind the star nut. I can only assume that these have been dropped in during the bike build. Can't think that there would be any other reason unless I'm missing something? The headset has cartridge bearings and they are present and correct.

There should not be anything but the star nut in the actual fork steerer. The headset bearings are supposed to be in the headset cups, which are on the frame.
I dont see how there could be anything behind the star nut, because the bottom of the fork steerer is hollow so anything behind the star nut will fall through the steerer and onto the tyre.0
