622x12c rim with 700x35 tyre issue

I've been having far to many punctures on my 6 month old Focus Mares AX 3.0 bike. I had 2 punctures yesterday on one 80k ride! There are a couple of very small tears in the tire but I don’t think this is the only issue.

Looking at a chart (http://sheldonbrown.com/tire-sizing.html) comparing rim and tyre size, my 13mm rim is not suitable with the 35mm wide tyres supplied with the bike(Continental Cyclocross Speed). Could this be the cause of my issues?

Is this chart compatible with a cross bike? Could it be a wrong sticker on the rims? Any help would be great! Thanks.

    What kind of punctures are they? Where are the holes in the tube? Are they pinch flats or pointy objects, abrasions or rim tape related?

    12mm is very narrow for a road rim - more typical is 16/17mm - have you measured the internal rim width? I'd check the rim tape - cheap, hard plastic rim tapes are notorious for shredding tyres. Replace with a softer plastic one or Velox cloth.
    Make mine an Italian, with Campagnolo on the side..
