Does the threaded adapter screw onto the small thread just under the valve?

    Yes - and if you do it up too tight it'll almost certainly unscrew the valve core and remove it when you take it off! So tighten the pump a little at a time, and keep pumping a bit to see when it's holding air, then don't tighten it any more!

    do up the valve cores *tight* (use a core tool or pliers)

    otherwise as above the core can unscrew

    the other thing to watch out for is that the thread inside the pump head can wear out with a lot of use, stranded with a flat tyre is no time to discover this, you can bodge it by wrapping a bit of plastic tape around the valve to bulk it up, but you need to hold the pump head in place, which ideally needs three hands
