Cable tension, chain wear, sprocket wear. It is probably cable tension.

I've had this before and realised later I'd over-adjusted the rear mech by a whole gear, so the easy gear was 2nd and a redundant click in top (barely noticable). Thing is I then adjusted the top stop so that I could get gear 1 and it does give a (slightly rattley) set of gears 1-3-4-5-6-7-8-9-9. Try readjusting the gears from scratch baring this in mind.0
