Gears skipping on new chain, and front mech shifting probs

I changed my chain (used existing cassette). The chain now skips on 2 or 3 gears whether in middle ring or big ring. I made sure the chain was the same length as the previous one.
As it's only happening in a couple of gears (both in middle and big ring), would this suggest that it's a worn cassette?
Also the shifting on my front mech is a problem going from big to middle ring. It changes fine going from middle to granny ring. It also goes fine from granny to middle to big ring. Only problem is big to middle ring. It takes a while to drop down sometimes (quickest way for me to change it is to drop down to granny ring then go back up).
In all the setup guides I've read, you setup the low screw so it throws the chain from middle to granny, and the high screw to go up to big from middle. My mech does this but doesn't go from big to middle easily.
Any ideas?

Sounds like the cassette be worn - but try and fine tune the front gears by adjusting cable tension.0

How did you join the 2 ends of the chain up?
Assuming you didn't you a SRAM power link (or similar), Is it a stiff link where you joined it?
A worn cassette is usually the problem but somebody beat me to it.0
