Fork Maxle keeps coming loose..

The maxle on my domains keeps loosening off, not too sure why and only seems to have been on the last 2 rides..
I've checked the little grub screw and adjusted it so its tight enough that I can still close it, but it seems to either just pop open or loosen as I'm riding!!
Last week, the maxle had worked its self about 1cm out of the forks!!!
Any ideas?

Check the compression expander/s are locking it0
