I had some custom cycling kit made about 5 years ago at http://www.owayo.com/cycling.htm Was pretty good.

How custom? I can thoroughly recommend Image Scotland who did the printing on the Gore jerseys for Teenage Cancer Trust (they are Gore's preferred supplier)

+1 for Owayo. I'm sure there are others, but found them good to deal with and was happy with the end product (save for the fact that it looks like it was designed by a committee of chimps, but can't blame them for that!)
