Cleat positioning advice (bit of pain)

I've ran all manor of SPD and SPD-SL cleats on my shoes for 2 years now and recently while using SPD-SL plastic cleats, I've started getting a *slight* bit of pain in the forward portion of my little toes metatarsal. (adjacent to the ball of my foot)
What would be the rough area of re-alignment be until I can get a proper cleat fit? I'm guessing to effectively align the cleat so my foot moves more inward to the crank. Both of my shoes are bang on centralised in terms of cleat positioning (my shoes have cleat guide lines). I have no problems with my left foot at all.
