Gear/chain trouble

I have shimano gears and I was having a delay between me changing gear and it actually shifting. I looked on you tube and watched how to adjust the tension to fix it. (turn adjuster antioclockwise until bike shifts, then turn back clockwise for half the amount of anti clockwise turns) Apart from a few semi turns I think I have it back to normal, apart form when I try to go in to lightest and second lightest gear on cassette (larger rings) Would it be coincidence this happened after shifting problems and my chain is worn? Or does my gears still need a bit of adjusting?

The changes arent slow anymore. the chain does not want to stay on the lower gears. its like its trying to jump out by itself, and makes a lot of clicking noisesCube Attain SL Disc

maybe a small amount of slack on the cable is needed?
Best thing to do I find is start from scratch.
Undo cable -
set cable tightner to max loosness
find top and bottom limit by hand moving the rear mech
re connect cable (make sure you are on smallest ring for chain and shifter)
Then just fine tune top and bottom limit (if needed and cable tension)
With cable tensioner you turn it anti clockwise to make the cable tighter0
