Filling in Ombudsman PPI form, right way/wrong way?

Wasn't sure best forum to stick it in, so here goes..
Currently trying to get my PPI back from MBNA for my credit card. I've already sucessfully done it myself for the loan, couldn't believe how easy it was..
Rang MBNA..
me - hi, had some loans, not sure if I had PPI
mbna - hi, yes you had PPI
me - am i entitled to any refund
mbna - yes, we'll work it out and respond within 28 days
true to their word, in not much after a month, I had had a couple of letters, NO questionaires or anything, then got a cheque through the post. Honestly, it was that simple
However, at the same time I was on the phone, they put me through to the credit card team as I also have a card with them. Conversation went similar, however they said they'd send me a questionaire.
Got the questionaire, but before i'd even had time to fill it in and return (they said return it within 21 days) they sent me a letter saying seeing as I hadn't returned the questionaire, they looked at the details they had and I was not entitiled to any refund, sorry.
So, quickly sent off the questionaire. They promptly replied again saying they had received it but their decision was unchanged.
Had a quick google and it seems that the MBNA credit card guys are still trying it on, as they've done it to others, who then when they go to FOS, they then pay up.
Spoke to FOS last week, they asked a few questions over the phone and have sent me a questionaire too. They stated in the letter that based on the very brief chat I had with the FOS, they couldn't determine if I was entitled to a claim or not, but to persue it.
SO... (sorry to waffle on....) the main question is.. are there certain answers or things to say on the form (bear in mind that my credit card was taken out in 1997 so I have no idea what was said at all at the time) which will stand me in good stead for a claim??
I wouldn't normally ask anything like this, but seeing as it seems like they are trying it on and have had a LOT of money off me over the last 15yrs, I should certainly do what I can to get it back.

That guy's paid far too much ppi looking at the state of his socks. Should be in for a good payout though.

I only read the first bit of the title, let's get this straight, you have an ombudsman you want to fill in and need advice? Surely as this is the Crudcatcher there can't be anything else to this thread?0

I stupidly put it in here as this is where kaise brought it to peoples attention of his success and it was talking to him that helped me out. So thought here might get the audience / other people that were actually interested or took action of a similar matter.
Probably better if it could be moved to the hub? Mods?0
