So *that's* why the barrel adjuster's not working...
...turn it as much as I like, no movement from the rear derailleur. Disconnect the cable and all becomes clear; at some point it's been turned so far anticlockwise it's not in the thread on the mech at all, just resting against it held there by the tension in the cable.
