I've spent much of the afternoon trying to index my gears having fitted some new bars (old bars had noodles holding the cable outers meaning I had to remove/refit the cables too).
Nightmare; skipping around all over the place, double shifting etc. I've fitted/maintained a few drivetrains on different bikes so thought I had this stuff sussed!
Went to the LBS to get a new cable as the old one wasn't exactly old but it did have a couple of kinks in it and so this seemed a likely culprit. Outers are only a few months old and looked fine on inspection. Didn't make any difference. So started thinking is the mech buggered? Is the LH shifter buggered? (Campag centaur so not the most expensive but not exactly cheap to replace either).
Went and had a sit down and a cuppa, went back out and started to inspect everything thoroughly. Noticed the pinch bolt on the rear mech looked a bit funny. Took it off and seated the cable correctly- perfect shifting.
