Crank arm nioses and it loosens and falls off!

Hi,
IN the last 6 months I have been getting really bad noises, creeks, clunks etc coming from somewhere around the spindle connecting to the crankshaft. Every time I put pressure down on the left hand pedal, either the crank arm where it connects to the spindle or the spindle creeks, clunks real loud and the crank arm works itself loose and falls off.
Can anyone help explain what is wrong and how to fix it?

Riding with lose cranks kills them.
You will most likely need new cranks now as you have damaged them."Do not follow where the path may lead, Go instead where there is no path, and Leave a Trail."
