Post mount and IS mount... whats the difference?

Hi all
The subject probably says it all, but I'm looking to get some new brakes. Can anyone tell me what they mean by post mount and IS mount and what I would need for my bike?... Cannondale SL4 with RST Omega fork.
It has Tektro Dracos on there at the moment - not sure if that carries any significance or not.

Just to add that I have learnt the hard way that converting from one fitting to another isn't always quite as simple as you think..... It's worth checking to make sure that the relevant adapter for what you want to do is available and that you're happy with any effects (ie increasing the rotor size etc).
I now know far more about brakes and adapters than is normal for a nice girlLife is not measured by the number of breaths you take, but by the number of moments that take your breath away....
