Can't get gears indexed....

I know the basics of setting up rear mech and indexing your gears (watched quite a few online video guides), but for the life of me I can't get mine set up properly for love nor money. I can use all of the gears but often it takes 2 clicks to get the rear derailleur to drop a gear... especially from the largest sprocket.
I've been tweaking the tension using the barrel adjuster and I just can't get the damn things to click smoothly down the gears ie from largest sprocket to smallest. Are there any obvious things I might have missed. The spring on the derailleur seems to move freely enough. Could there be a lack of tension in the spring? Or maybe it's time for new cables? these haven't been changed/cleaned in 3 years.
Or could the mech hanger be bent?
