Worn races and bearings

Hi,
after my rear axle started coming loose again and starting to feel rough, I gave the hub a bit of a service. Unfortunately the bearings and the races on the drive side are worn, as muck had got in there. It's a 'formula alloy disc' hub that came with the Trance X4, with a Deore cassette.
I've only ever serviced the hubs before, and never found a problem, so unsure on the replacement.
Do I need to replace the freehub? If I replace that, will it come with replacement bearings, and races (the one on the axle is damaged too).
