Pedals / cleats

Hi
I have a question on pedals / cleats. I've recently ungraded my bike and would in ideal world like you now use cleats.
However a knee operation a couple of years ago (ACL & cartilage) made using toe clips painful when used on my old bike. I believe the problem was because the toe clips forced by feet to be aligned straight ahead. When I ride without top clips normally, my right foot tilts out to the side a little, about 20-30 degrees clockwise. I'm under the impression that you can adjust the angle of of the foot in the cleats but worry that having my foot fixed would cause the same problem as the toe clips did.
I understand this a quite a personal problem but wondered if anyone has been in a similar situation or has and info on resources which could help?
Thanks in advance!

After doing a bit of research it seems there is loads of info on these message boards already, regarding this topic. I should have checked that first, apologies.
My next question is, does anyone know of shop in London where the staff are fairly knowledgeable about fitting. Preferably central, north or east.

Cyclefit are very knowledgeable, pretty expensive and their notion of bike fit probably doesn't suit everyone. Fine if you're the proverbial 'fat dentist''. One option woud be to go with a clipless pedal with lots of free-float like Speedplay and then seek the advise of a bike fit specialist if you still can't get on with them.Make mine an Italian, with Campagnolo on the side..0

Haha thanks, I'm not sure what a proverbial fat dentist is but I don't think its me...
Yes, Speedplay seem to be a popular option for people with knee problems. Good advice to see how I get on before spending a fortune on fitting!0
