Presta valve - inflate to high pressure but air escaping?!

Hi all - when i attempt to put a over 40 psi in the my presta valve with hand pump the air starts to escape from pump/pump attachement? it seems i get to a certain pressure with out any escaping air then i can hear with every additional pump air escaping (not a puncture - just as tho the tube won't take anymore is dispelling the additional air?).
I thought my pump might be at fault so have just bought a brand new one but same problem. Also I have tried a new inner tube. So it must be soemthing i am doing wrong!
help me pls getting annoyed with punctures from under inflation!

must be operator- as i said new inner tube so can't be the valve.! thanks for reply0

buy a decent upright track pump and you wont have a problem0

Is it a tube with a removable valve-core (conti ones are like this generally)? If so it could be loose.0
