Is this terminal?

Bottom bracket; grasping each pedal and pushing them the BB is going clonk clonk.

Is it time for a new BB or do the end caps just need tightening?

    most likely new BB.

    but depending on the set up something may just need tightening.

    Take the chain off and spin the cranks by hand slowly. You'll feel any grittiness in the bearings. If they feel more like they are grinding than turning then it's time to change the bottom bracket.

    If there's excessive play then also it's time to change it.

    that'll bring me up to £350 this month on bike bits (frame, fork, bar tape, cables, disc pads etc), who said it was cheaper than a car?

    Oh well, I suppose 15k miles from a BB isn't bad.
