trek disc brake adapter

Does anyone know where i can get a disc brake adapter for my beloved trek 4000? Have seen that some newer treks have a small bracket bolted to the chain stay. Any info gratefully recieved.

Have tried my lbs they can't seem to find it in their catalogue. Do you know where i can get one from, part number or anything? Have seen the A2Z one but it isnt quite as subtle as the trek one. Many thanks for any info.0
