Worn Cassette

Hi,
A newbie to the forum here and I've searched the forums.
I have a problem with the chain slipping on the smallest cog on the rear cassette, I've attached a a picture of it, do you think it's worn and thus causing the slipping, if so do I need to replace the chain as well as the cassette?
I've already replaced the rear mech hanger so I've ruled a bent hanger out.

Assuming the gears are set: well, it does look a little worn. Some of the teeth are certianly showing wear. Is this an often used gear?0

check your limit screws on the rear mech. Check the B-screw, and adjust it so that the top jockey wheel is pretty close to the cassette.
Failing that, check your dropout for alignment, it might have taken a knock.
That sprocket is a bit worn, but I've definitely seen much worse still functioning fine.0
