Problem changing up with Campag

Guys,
Last couple of times I've been out on my bike I've noticed that my gear change is a bit off on the rear.
Mainly, if I try and change up two gears with the big shift on my Campag groupset it seems to initially change by two sprockets, and then jumps down one.
Any ideas what the problems could be? I've tried more tension in the cable and that doesn't seem to have resolved the problem.
