Where can a get one from please? a empty one of course, dont want to be borrowing someones!

    My mate signed his own last year as his doctor was going to charge him £75 for the privilege. They hardly look at them when you complete your registartion at the top of ADH.

    Mine didn't. But then my doctor is a friend.

    I photocopy the one I had from 2006 - and just change the date every year.

    Seems to work.
