Shifting problem

I'm first to admit I'm clueless with gears. It's always a part of my bike I’ve avoided but determined to fix that..
However, a few months ago I was struggling to shift from top to bottom at the rear. I adjusted the barrel and it seemed to do the trick. The problem returned today though. Am I right in thinking I turn it clockwise to help shift from largest to smallest? Is that tightening or loosening? If it's tightening is it worth just pulling the cable through more?

Depends on the direction of the mech, top normal or bottom normal.
It sounds an odd problem for a normal rear mech, as usually the cable will stretch and loosen, where as it seems yours is tightening!
You may have a rapid rise mech.0 -
also check for sticky cables as this is the main culprit for malfunctioning gears.Viner Salviati

But what do you call top and bottom. Top is small and bottom is large.
Clockwise will loosen the cable.I don't do smileys.
