it appears Sven Nys' pedal wasn't fitted correctly........not quite sure how you could fit one incorrectly though?

Probably a standard case of taking the bike out of the luggage bike case and forgetting to tighten the pedals when putting the bike together... it happens... no point blaming shimano for that one.**************************************************

cranks look intact to me and non-drive side (the missing pedal side) is clean... i.e. the pedal fell off cos it wasnt screwed in :-) If it snapped, surely some of the spindle would be left intact and attached to the crank arm... I bet if you look at his left shoe he still had a pedal attached (but cant see from the angle of the pic above)!
BTW: A pedal failure would not enable the 10mm of threaded bolt on the axle to screw out from the crank arm.... human error. full stop.**************************************************

it is a bit strange for it too fall off even if it wasn't screwed on properly as the thread screw in the same way you pedal so in theory should only get tighter....?0 -

If It hadn't caught the thread properly but had gone in a touch wonky then the thread could have been stripped causing the pedal to pop out. No amount of cycling would have tightened it.Warning No formatter is installed for the format0 -
