Handlebar sizing - advice please

I need to swap over to larger handlebars but am unsure how they're measured. Is it from centre to centre of the bar's diameter, or outermost end to end?

normally its centre to centre but some manufactures (deda for one)
do it outside to outside

Thanks for the reply. I guess that makes my Specialized ones 42cm then - not that great for 48cm shoulders.

Also some are measured across the bottom of the drops which flare out so the tops are 2cm narrower (ie FSA Omega) :?
