jumping gears

My commuter bike, a giant defy 3, keeps jumping two gears on the 6th & 7th gears. What do I do to correct this?

Any help appreciated.

    have you tried adjusting the derailleur? This is done by twisting the barrel where the shift cable abuts the rear mech cage. Normally twist anti-clockwise to increase tension (pull chain towards the wheel) and vice versa,

    Yes I've tried this and hasn't helped. I have followed a tutorial from an on-line video and re-set the whole system. The shifters are fine throughout the gears up and down and on all three rings (it's a triple) but when I'm riding and changing up or down I have to repeat the process to get it to work on these particular gears. I've had the bike around 6 months so I doubt the gears are worn! Weird
